(ii) Operation Room Assistant Course from a recognized institution i.e. ICTE/State Board of Technical Education/Indian Medical Association/Christian College of Nursing (St. Stephen Hospital)/ Universities approved by UGC/Delhi Govt./Central Govt./State Govt
Experience Requirements: 05 years experience as OT Assistant in Operation Theatre/CTS/Neuro surgery/gastro surgery /CSSD/ Anaesthesia/ Gas Plant/ Anaesthesia Workshop/ICU/Surgical/ Resuscitation in a recognized Institution/hospital.

NOTICE
Application are invited from External Candidates who have passed final BDS Examination in the year 2012 and wish to do internship at Maulana Azad Institute of Dental Sciences, New Delhi against the vacant post. Application along with the following documents may be submitted to Registrar of Maulana Azad Institute of Dental Sciences, New Delhi as per schedule below:-
LIST OF DOCUMENTS TO BE ATTACHED
1. Application by the candidate giving reasons to do internship at Maulana Azad Institute of Dental Sciences.
2. No Objection Certificate from his/her passing out college stating that the college was not derecognized by Dental Council of India during the course of training from admission to his/her passing out and the college is duly recognized by Dental Council of India subject to selection at MAIDS.
3. Permission of the University to which the parent College is affiliated to do internship at MAIDS subject to selection.
4. Permission of the Dental Council of India to do internship at MAIDS subject to selection and certificate from Dental Council of India that Dental College/Institution where the student at present studying has been recognized by the Dental Council of India from the year the students was admitted in this Institution to the year of his/her passing out.
5. Character and conduct Certificate of the student from the parent College/Institution.
6. Attested photocopy of mark sheets of all the BDS examination (for each part/year) passed.
7. Attempt Certificate of passing BDS (each part/year).
8. Documentary evidence, distinction/academic Medal, if any awarded for any subject in University Examination.
9. An undertaking that the student is prepared to do internship without any pay, stipend or honorarium and will deposit Rs. 100,000/- (Rupees one Lakh) as a Fee if got selected for internship, in the form of Demand Draft in favour of “Director-Principal’’, MAIDS New Delhi, payable at New Delhi.
10. Undertaking that he/she will follow the Internship programme of Maulana Azad Institute of Dental Sciences, New Delhi.
11. Undertaking he/she would maintain good conduct, discipline and decorum of the Institution and the authorities of Maulana Azad Institute of Dental Sciences, New Delhi have the right to discontinue his/her internship at any time for his/her misconduct, indiscipline and unsatisfactory work.
12. Two passport size photographs, one affixed on the application form.
NOTE
1. Individual who have earlier submitted their request for internship have to apply afresh along-with the required documents as above with prescribed proforma.
2. Training Fees of Rs. 100,000/- (Rupees one Lakh) to be paid by the selected candidates before the start of internship.
3. Application without requisite documents as per notice will be summarily rejected and no extension will be granted for submission of any documents.
4. No application will be entertained after 15/11/2012 at 4.00 PM.
