HOW TO APPLY:-
1- Candidates should send the application in plain paper alongwith two self-signed recent
passport size photograph giving full details of 1. Advt. No. 2.Name of the post& Post Code,
3.Bank Draft (Number, Date, Amount & Name of issuing bank) 4. Name of the candidate,
5.Father’s/Husbands Name 6. Date of Birth, 7.Marital status (Single/Married), 8.Permanent
Address 9. Correspondence Address, 10. Name of the State (You belong originality), 11.
Category (in case of reserved category, attached certificate issued by the competent
authority), 12.Academic Qualification (Exam. Pass, year, Board/Univ., Division, percentage of
marks and subject), 13. Details of experience (Post held, Period of working, Scale of pay,
employer’s name and address, nature of duties), Signature of the candidate with date.
2- Self certified of testimonials/documents in support of age (date of birth), educational
qualification, experience and caste must be enclosed with the application.
3- Reservation is applicable for SC, ST and OBC of Uttar Pradesh state only. SC, ST and OBC
candidates of other states will be considered as general candidates. A bank draft Rs. 100/-
in case of SC,ST categories candidates of Uttar Pradesh State only and Rs. 250/- in case of
OBC and General categories candidates in favour of Director S.G.P.G.I.M.S. , Lucknow.
Payable at State Bank of India, SGPGIMS Branch, Lucknow (7789) must be enclosed along
with the application which is non-refundable.
4- Application should be addressed to the Director, Sanjay Gandhi Postgraduate Institute of
Medical Sciences, Raebareli Road, Lucknow – 226014 (U.P.) and should be send by
Registered Post /Speed Post only. By hand application will not be accepted.
5- Envelope should be super scribed with the advertisement No. and name of the post.
6- Director reserves the right to cancel the advertisement without assigning any reason and
in that case application fee will not be refunded.
7- Number of post may be increased/decreased depending upon the need of the Institute.
8- No TA/DA shall be admissible for attending the interview / test.
9- Applicant working in Govt./Semi Govt./Undertaking/Autonomous institute should apply
through proper channel or produce No Objection Certificate at the time of Interview / Test.
10- Candidates with higher qualifications shall be preferred.
11- Maximum age limit 40 years for General Candidate and 45 years for reserved candidates
(including relaxation period) reckoned as on 01.07.2012
12- Incomplete applications (i.e. without testimonials, document, bank draft, unsigned
Applications) and applications received in this Institute after the expiry of the last date will
be summarily rejected.
13- Merely fulfilling minimum requirement does not essentially qualify candidate to be called
for test/interview.
14- Reservation norms for category (SC/ST/OBC) will be determined as per notification of UP
Govt. only. Reservations for physically handicapped will be admissible as per U.P. Govt.
rules.
