DOCUMENTS REQUIRED FOR RENEWAL OF
REGISTRATION CERTIFICATE
1. File Cover of Card Board with tag.
2. SIX latest identical passport size Photograph (2 be duly attested).
3. Renewal Fee (For Five Years Only) through Bank Draft payable at
“Panchkula/Chandigarh” in favour of “REGISTRAR, HARYANA
STATE PHARMACY COUNCIL”. Fee Payable to be confirmed from
Council’s Office from time to time.
4. Prescribed application form & Form “L” duly filled.
5. Original Affidavit on non-judicial Stamp Paper of Rs. 10/- duly
attested by 1st Class Magistrate /Notary Public.
6. Attested copy of Matric Certificate showing Date of Birth.
7. Attested copy of last Fee paid Receipt issued by the Council’s Office.
8. One photocopy duly attested of the original Registration Certificate of
the Council.
9. Attested copy of Diploma OR Degree of Pharmacy on the basis of
which candidates was registered for the first time.
10. Attested copy of Ration Card (first & its back page) showing name &
address of applicant OR any other valid residence proof in Haryana
(i.e. Voter Identity Card, Passport, Adhar Card, Haryana Domicile
etc.)
11. Attested copy of at least two CME certificates (Refresher Course)
programmes.
